The Patient Health Questionnaire-2 (PHQ-2) is a brief two-question screening tool that can be used to quickly identify individuals who may be experiencing depression. The PHQ-2 asks about the frequency of depressed mood and anhedonia (loss of interest or pleasure) over the past two weeks. A score of 3 or higher on the PHQ-2 indicates that further evaluation is needed. If the PHQ-2 is positive, the Patient Health Questionnaire-9 (PHQ-9) can be used to assess the severity of depression. The PHQ-9 asks about nine different symptoms of depression, including depressed mood, anhedonia, fatigue, sleep disturbances, appetite changes, difficulty concentrating, psychomotor agitation or retardation, and thoughts of death or suicide. A score of 5 or higher on the PHQ-9 indicates mild depression, a score of 10 or higher indicates moderate depression, and a score of 15 or higher indicates severe depression.
